Forms and templates

The following forms and templates are available to view or download for all FedNor clients. If you need help selecting a form or template for your project reporting, please reach out to your local FedNor Officer or call us toll-free at 1-877-333-6673.

Tip:

Save the application link to your desktop or to any location you would normally use.

Then open up Adobe Acrobat Reader.

Within Adobe Acrobat Reader, select Open, then browse to your saved location, and then open the downloaded file.

The application should open up correctly.

Claim Summary and/or Advance Claim Form

The Claim Summary and/or Advance Claim Form is required to request advances and/or payment for the eligible supported costs at the intervals outlined in the project's contribution agreement.

Cost Forecast

The Cost Forecast template is required to estimate the funding disbursement agreed to as per the project's contribution agreement.

Detailed Transaction List

The Detailed Transaction List template is required to report on the eligible supported costs incurred during the period of a claim. All transactions are to be listed by eligible supported cost categories itemized as per the project's contribution agreement.

Direct Labour Overview

The Direct Labour Overview template is required to report the details of the positions hired, hours worked, rate of pay, benefits etc. to be used in claiming eligible labour costs as per the project's contribution agreement. Alternative computerized accounting reports with the required information may be used in lieu of this form.

Capital Equipment Acquisition

The Capital Equipment Acquisition template is required to list capital equipment purchases when completing a claim with eligible capital costs.

Recipient Claim Certificate

The Recipient Claim Certificate form is required to certify that the recipient's authorized representative has examined the FedNor contribution agreement and related documents and information required to complete a claim request.

Final Statement of Total Project Costs & Funding (Actual)

The Statement of Total Project Costs and Funding (Actual) form is required to report on the total incurred costs and all related funding sources at the end of a project.

Request for Payment by Electronic Deposit Interchange (EDI)

The Electronic Deposit Interchange (EDI) form is required to allow for payments to be processed by direct deposit to the recipient's financial institution.

Progress Activity Report

The Progress Activity Report form is required to assess the progress and the compliance with the terms and conditions set out in the project's contribution agreement.

Final Results/Two-year Follow-up Results Report

The Final Results Report forms are required as per the contribution agreement. They are to report on the outcomes and impacts of the project for evaluation purposes.

Ultimate Recipient Report (Further Distribution)

The Ultimate Recipient Report (Further Distribution) form is required to report on the funding allocations made through a third party and confirm the recipient/project details are in compliance with the terms and conditions set out in the project's contribution agreement.

Intern Certification (Youth Internship)

The Intern Certification form is required to be completed once an intern has been hired to confirm eligibility for funding.

Employer Exit Interview (Youth Internship)

The Employer Exit Interview form is required to be completed at the end of the internship by a representative of the recipient. The information provided is for program evaluation purposes only.

Intern Exit Interview (Youth Internship)

The Intern Exit Interview form is required to be completed at the end of the internship by the intern. The information provided is for program evaluation purposes only.
